What Is an Endodontic Retreatment?

Endodontic retreatment involves performing an additional endodontic procedure (i.e. a root canal) on a tooth that has already received one. For the patient, the experience in the chair is essentially the same and should ideally have the same result—no more pain and a preserved natural tooth.

When Is Endodontic Retreatment Necessary?

Endodontic retreatment is typically recommended if a tooth that has previously received a root canal starts to become sore or painful again. This is likely because it has developed a new infection or decay, OR the initial infection was not completely removed the first time.

  • First, your endodontist will discuss treatment options. If retreatment is chosen, they will reopen your tooth to access the root canal filling. In some cases, crowns, posts, or other restorative materials must be removed for proper access.
  • Once the filling is removed, the endodontist will clean the canals and carefully examine the tooth under magnification, identifying any hidden canals or unusual anatomy needing treatment.
  • After thorough cleaning, the canals are filled and sealed, and a temporary filling is placed. If canals are too narrow or blocked, endodontic surgery may be necessary to seal the root from the other end.
  • Once retreatment is complete, you must visit your dentist promptly for a new crown or restoration to protect and fully restore your tooth’s function.

Are There Any Alternatives to Endodontic Retreatment?

The most common alternative to endodontic retreatment is tooth extraction, but this is something we would rather avoid in most situations. That’s because it’s always beneficial to maintain your natural teeth, as they are stronger and more durable than even the highest quality dental restoration. Plus, the expense of removing and replacing a tooth will always be higher compared to endodontic retreatment.
